Specimen Type: Infecting bacteriaContainer/Tube: Slant, agar plate, or tape plate
Collection Instructions:
1. Specimen source is required.
2. Organism must be in pure culture, actively growing.
3. Place specimen in biohazard bag (double bagging preferred) and label as an etiologic agent.
4. Label specimen with patient’s name (first and last), specimen source, and date and time of collection.
Additional Information: Conventional organism identification will be performed and antimicrobial agents appropriate to the organism and specimen source will be tested, if applicable.
